Job Overview
Sterling Hospice is looking for you to join our compassionate team as a Hospice After-Hours Registered Nurse (RN), providing critical after-hours clinical support to patients and families during evenings, nights, weekends, and holidays on a 7-day on / 7-day off schedule. This energetic and dedicated role offers an opportunity to deliver high-quality, patient-centered care in a dynamic environment. As an integral part of our hospice team, you will assess patient needs, manage symptoms, and coordinate care to ensure comfort and dignity at the end of life. Your expertise will make a meaningful difference for those facing life-limiting illnesses during their most vulnerable moments.
Location: Based in the Garland area (serving 50-mile radius)
Responsibilities
• Respond promptly to after-hours patient calls, providing expert triage and clinical guidance to address urgent symptoms or concerns.
• Conduct comprehensive patient assessments in-home visits when necessary, utilizing your knowledge of physiology and medical terminology.
• Manage symptom control including pain management, shortness of breath, nausea, and other comfort measures aligned with hospice care standards.
• Collaborate with interdisciplinary teams to coordinate care plans, medication adjustments, and necessary interventions while adhering to HIPAA regulations.
• Document all patient interactions accurately within electronic health record (EHR) system WellSky Consolo, ensuring compliance with NCQA standards.
• Educate patients and families on medication administration, advanced directives, and end-of-life care options with sensitivity and clarity.
• Maintain readiness for emergency situations by utilizing aseptic technique and airway management skills when needed.
• Conduct after-hours admissions when needed.

Requirements
• Valid RN license with current state licensure; experience in hospice required.
• Demonstrated critical care experience utilizing your critical thinking skills to resolve complex patient issues after-hours in their home.
• Knowledge of Hospice goals of care, medication administration, IV infusion techniques, and catheter care.
• Experience working with individuals with disabilities, memory care needs, or in long-term care environments like nursing homes or assisted living facilities.
• Ability to handle complex cases involving acute pain management and infectious disease care with professionalism and compassion.
• Excellent patient assessment skills combined with the ability to work independently during after-hours shifts.
• Strong understanding of HIPAA compliance and infection control practices; capable of managing stressful situations calmly. The TNMHPO Education Subscription is for agencies who wish to provide on-demand education for the entire staff. This subscription is an add-on to a TNMHPO membership and can’t be purchased separately.

A TNMHPO Membership Plus Education Subscription gives members of the Texas New Mexico Hospice & Palliative Care Organization unlimited access to all archived online education for the entire year.  You can provide training to your current staff and utilize these educational opportunities for new employee orientation.  There are courses for every discipline on your hospice team and continuing education units and proof of attendance for Administrators, Nurses, Social Workers, and LPCs.

The optional education subscription may only be used by staff of the joining hospice agency, not to train partners, affiliates or sister organization staff. If your hospice is part of a home health, then the home health division will need to pay its own dues and education. 

TNMHPO reserves the right to limit users if we feel the system is being misused.
