As we continue to grow, we are seeking an experienced hospice professional to serve as our Clinical Excellence Coordinator, a key leadership position focused on strengthening clinical excellence, developing team members, supporting quality initiatives, and helping ensure we continue delivering outstanding care to every patient and family we serve.

Position Summary

The Clinical Excellence Coordinator works closely with our Director of Operations and clinical leadership team to strengthen quality, documentation excellence, regulatory readiness, and employee development throughout the organization.

This individual will help identify opportunities for improvement through chart audits, documentation reviews, quality monitoring activities, and trend analysis. They will then develop and deliver meaningful education, coaching, and support to help our team continuously improve.

The ideal candidate is passionate about hospice care, enjoys developing others, and understands that education is most effective when it is connected directly to real-world clinical practice.

This position is based in our Mansfield office and participates in a four-person Administrator-On-Call rotation.

Example Responsibilities Could Include:

Orientation and onboarding of new clinical staff
Annual competency assessments
Ongoing education and in-services
QAPI program coordination and data analysis
Chart audits and documentation improvement
Regulatory readiness (ACHC/Joint Commission/CMS)
EMR training and support
Assisting the DOH with clinical tasks and scheduling management or other projects as needed.
QualificationsRequired

Active Texas Registered Nurse (RN) license in good standing
Minimum of five years of hospice experience
Strong understanding of hospice regulations, eligibility requirements, and documentation standards
Exceptional communication, presentation, and coaching skills
Strong organizational skills and attention to detail
Ability to coach, mentor, and develop others
Ability to work collaboratively with interdisciplinary teams
Proficiency with electronic medical records and Microsoft Office applications
Preferred

Previous experience as a Hospice Clinical Manager, Clinical Educator, or other hospice leadership role
Experience with QAPI, compliance, quality improvement, or survey readiness
Experience conducting chart audits and documentation reviews
CHPN certification
Experience preparing for or participating in state, ACHC, Joint Commission, or CMS surveys
