Individuals who are nominated for the awards must be actively involved in the hospice movement in Texas or New Mexico.

Nominate a Hospice Professional
The Awards Committee will award based on the strength of the nominee’s accomplishments and will consider the wide variety of professional roles to demonstrate the core value of teamwork. Use this form to describe nominee’s accomplishments and contributions to hospice, include impact of service to hospice and patients, dedication and commitment, to goals, philosophy and stewardship of hospice, initiative, dependability and reliability. Nominations are due by January 12, 2024.
