HR Coordinator

JOB SUMMARY:
The primary functions of the Nurse Practitioner are to assist in overseeing the clinical care and treatment of patients and families as well as consulting with the patient’s attending physician. Provide clinical direction with the Interdisciplinary team to ensure quality care along with the ability to explain plans and goals to others. Displaying knowledge and understanding of the Agency’s policies, procedures, and philosophies is a must.
Additional responsibilities include:
• Completes face-to-face encounters with patients, as assigned, prior to 3rd benefit period and each period to follow under the direction of a hospice physician.
• Serves as attending physician upon request.
• Consults with and educates patient/family on the disease process, self-care, prevention strategies, end-of-life care, and physical/psychological aspects.
• Collaborates with IDT’s development of patient/family plan of care to assure quality care.
• Provides clinical assessment results to the hospice physician after face-to-face encounters.
• Performs and documents visits to patients/family in a timely.
• Maintains close communication with patient’s physician, reporting all changes in status and collaborates with the physician to develop treatment protocols.
• Assists with admissions, on-call, staff, and community education.
• Maintain the patient’s and family’s right to privacy and confidentiality at all times.
• Works non-judgmentally with patients and families of various cultures, religious, and socioeconomic backgrounds and lifestyles.
• Attends weekly IDT meetings.
• Treats patients, families, and staff with respect and compassion.REQUIREMENTS:
Current license to practice as an RN in the State of Texas and as an Advanced Practice Nurse. Minimum of 2 years of medical-surgical or home health/hospice experience preferred.
