HOSPICE RN On-Call – DFW Area

** Looking at areas surrounding DFW AREA and surrounding counties.

** Lewisville, Carrollton, Farmers Branch, The Colony, Frisco , Irving, Dallas

We Are looking for you ! Case Manager. full-time. Interviews are IMMEDIATE as we are hiring as quickly as possible.

· Current license as a Registered Nurse.

· Experience in a hospice a must.

* Be On Call for our patients and their Needs.

· Excellent decision making skills, and communication skills.

· Kinnser experience is a plus.

· Requires walking, pushing, pulling, and lifting up to 40 pounds

Responsibilities:

· Provide skilled professional care visits to patient and family members after hours week nights and weekends.

· Work closely with DON and IDT members to ensure proper follow up care from previous night.

· Timely documentation within 24 hours.

· Will provide direct patient care
