Please use the "Find a Hospice" locater on the side bar of this page to find out if your Hospice is a member. If your hospice is a member then you may register as a member. To register on-line you MUST pay with Paypal (note: you do not have to make a PayPal account to pay, there is an option to pay without logging in) . If you are registering for multiple attendees, complete this form for the first attendee and then send the rest of the names in the special instructions/notes when checking out with Paypal and also change the quantity of conference attendees in the shopping cart in Paypal to match the number of attendees.
